i need a script

We have a tendancy to use the old standard: "Now. I want to remind you about the survey you will be getting. If there is a reason you can't give us extremely satisfied - please let me know now so I can fix whatever it is!" We do it again on follow up calls and make sure the expectations for the survey are high marks. We still miss points, but the responses have definitely been better lately.
